Please include the merchandise to be autographed with your request. (1 Item Only) Please allow 4-6 weeks for processing.
All Charitable organizations must send a copy of their 501 © (3) status issued by the IRS with their request.
All merchandise autographed must be electronically protected and registered by PROVA GROUP, INC to safeguard its authenticity.
The fee for the Authentication is $20.00 and must be sent with the request. Download the following request form and send in along with the $20 and the item you would like autographed:
