• Applications due: June 15, 2013
• Grant decisions made: July 9, 2013
• Notices to applicants: July 30, 2013
• Distribution of funds: October 2, 2013

Please submit your request to smithcharities@gmail.com and include the following information:

• Description of program or experience
• Amount requested
• Description of target population including geographic location
• Number of children who will benefit
• Website of program if applicable
• Contact name including title, email and phone number
• 501(c)(3) documentation
• Most recent Tax Form 990
• List of board members
